Wrist fractures are among the most common fractures of the upper limb. In recent years, the management principles of these fractures have been widely challenged and now require an anatomical reduction followed by early rehabilitation and mobilization, with very little immobilization. As a result, orthopedic treatment has evolved towards specific surgical treatment adapted to the fracture type, the patient’s anatomy, and the preservation of surrounding tissues. Locked anatomical plate osteosynthesis has thus become a standard, especially for treating the most complex fractures, to achieve stable anatomical reduction and prevent joint incongruence responsible for post-traumatic arthritis.

The Xpert Wrist range was designed according to these principles, offering a complete range of anatomical implants for the treatment of intra- or extra-articular wrist fractures and malunions. It incorporates locking technologies using locked screws, locked peg screws (cannulated or not), or locked pins, as well as a wide variety of instruments to support the surgical procedure.

The implants in the Xpert Wrist range are dedicated to the fixation of fractures, osteotomies, and arthrodeses of the hand and forearm in adults.

VOLAR – XS SIZES

The extra-short volar plate has been specially designed for minimally invasive management of simple extra-articular distal radius fractures:

  • Compact design allowing a minimally invasive approach and a 1.5 cm incision
  • Extra-short plate specifically designed for the MIPO technique, preserving the pronator quadratus muscle
  • Divergent screw orientation enhancing plate anchorage
  • Instruments dedicated to minimally invasive surgery (MIS) available for XS and size 1 plates (standard and narrow)

VOLAR – SIZES: 1, 2, 4, 5, AND 6

A complete range of volar plates with 3 head widths (narrow, standard, wide) has been developed to allow adapted screw positioning along the subchondral bone:

  • Preformed plates following the watershed line for anatomical fit
  • Slim profile and rounded edges limiting tendon contact
  • Polyaxial or monoaxial locking technology
  • Locked oblong peg technology (patented)
  • Window for better visualization of the fracture line or insertion of bone substitute

VOLAR – SIZE 3

The volar plate size 3 (narrow, standard, wide) is distinguished by its two pre-oriented locked monoaxial pegs, designed to reach the tip of the radial styloid and fix small fragments:

  • 2 pre-oriented locked monoaxial pegs targeting the radial styloid to reduce small fragments and support the radial column
  • Preformed plate following the watershed line for anatomical fit
  • Slim profile and rounded edges limiting tendon contact

VOLAR RIM

A complete range of VOLAR RIM plates with 3 head widths (narrow, standard, wide) has also been specifically developed for extra-distal fractures:

FRAGMENT SPECIFIC – RADIAL AND INTERMEDIATE COLUMN

A complete range of specific plates has been designed to treat complex distal radius fractures, fragment by fragment:

  • Dorso-medial plates (3 extra-narrow head sizes and 1 standard head size)
  • Dorso-lateral plates (1 size)
  • Distal radial column plates (3 sizes)
  • Latero-dorsal radial column plates (3 sizes)
  • Radial palmar plates (2 sizes) with two pre-oriented monoaxial locked pegs toward the radial styloid

FRAGMENT SPECIFIC – HOOKS

To complete our offer and provide a comprehensive solution for the treatment of complex distal radius fractures, we offer hook plates:

  • Anterior hook plates (1 symmetrical size and 1 left and right size):
    • Possible assembly of an anterior hook plate with a radial palmar plate to reach the radial styloid
  • Posterior hook plate (1 symmetrical size)
FRAGMENT SPECIFIC – DISTAL ULNA

Distal ulna plates are offered in 2 sizes and provide a treatment solution for fractures of the head, neck, and distal diaphysis of the ulna:

  • Preformed plates for anatomical fit
  • Polyaxial or monoaxial locking technology
  • Locked oblong peg technology (patented)
  • Anterolateral positioning ensured by an anterior tab
  • Hook facilitating plate positioning on the ulnar styloid
  • Bending zones to achieve the desired fit if needed

OSTEOTOMY PLATE

Specific features of the osteotomy plate, size 2:

  • 2 locked oblong pegs allowing adjustment of the plate positioning with a locked or non-locked screw.
  • Compatible with PSI

Specific instruments for this plate:

  • Distraction clamp to adjust ulnar variance
  • Drilling guide for the distraction clamp

The osteotomy plate and instruments are not included in the Xpert Wrist kit, they are ordered separately.

SPANNING PLATE

Spanning plates provide a provisional internal fixation solution for complex distal radius fractures, especially highly comminuted ones or for elderly patients with poor bone quality:

  • Symmetrical plates designed for the second or third metacarpal
  • Anatomical design following the curvature of the radial diaphysis
  • Rounded edges, smooth surface, and slim profile especially on the distal part of the plate to protect soft tissues
  • Locked and non-locked screws of 2.4 mm distally and 3.5 mm proximally
  • Oblong pegs for plate adjustment

Two designs and two plate sizes to fit different patient anatomies and meet surgical needs:

  • Curved plates at 12° for a neutral hand position
  • Straight plates
K-LOCK – PIN LOCKING FIXATION SYSTEM

The K-Lock is a pin locking fixation system on the plate, designed to secure unstable fragments or fragments too small to be fixed with locked screws. The pin locks directly into the plate using a nut. This locked pin system stabilizes small fragments without requiring drilling, which can cause harmful micro-fractures induced by vibrations.

 

Patented technology, the K-Lock is compatible with all plates in the Xpert Wrist range, whether Volar or Fragment Specific.

 

The K-Lock system instruments are available in the following configurations:

  • Optional Xpert Wrist kit
STEPWISE TECHNIQUE – SPECIFIC INSTRUMENTATION

The Xpert Wrist range also offers instrumentation dedicated to the treatment of intra-articular distal radius fractures.

 

The Stepwise technique is a fragment-by-fragment reduction method, including intra-articular fragments, which can be performed under arthroscopic control using dedicated instrumentation:

 

  • Short and long pin guides
  • Instruments and cannulated screws

How to use the instrumentation?

  • Reduction and fixation of fragments with pins, using short or long guides depending on the pin length
  • One-by-one replacement of the pins with cannulated screws or locking nuts (K-Lock system)
